Migraine - If you get migraine headaches and are currently using a medication that conatins butalbital (Fioricet, Fiorinal) you may qualify.

Sinusitis - If you have a new sinus infection, you may qualify.

Multiple Sclerosis - If you have had or were just diagnosed with MS and will be starting therapy with Betaseron, you may qualify.

DVT / PE - If you had a deep venous thrombosis (blood clot in your leg) or a pulmonary embolism (blood clot in your lung) and have been taking Coumadin (warfarin) for 6 to 18 months, you may qualify.

High Blood Pressure - If you are on at least three different medications to help control your blood pressure, but the top number is still above 130, you may qualify

Atrial Fibrillation - If you get palpitations or a fluttering feeling in your chest, or have other symptoms of this heart rhythm and it comes and goes (not chronic AFib), you may qualify.

COPD (chronic bronchitis and/or emphysema) - If you have a chronic cough are a current or ex-smoker, you may qualify.

ASTHMA - If you are between the ages of 18 and 75 and have been unable to control your allergic asthma with standard treatments like Advair 500/50, you may qualify.

INSOMNIA - Do you have trouble sleeping?  If you are between 18 and 64 years old and are experiencing difficulty falling & staying asleep, you may qualify.
